Join us on for "Paving a Path to Optimize the Role of Community Health Workers", the first conference of the New York State Community Health Worker Association.  

The conference aims to bring together community health workers (CHWs) from across the state along with health care leaders, policy makers and representatives from the public and non-profit sectors, to discuss opportunities to support and sustain the role CHWs play in improving health outcomes for New Yorkers.

During the conference, the Initiative will release a summary report, "Paving a Path to Optimize the Role of Community Health Workers" outlining recommendations related to CHW scope of practice, training and credentialing, and CHW financing. 

The conference will feature a panel discussion outlining how CHWs can be integrated into evolving health reform innovations such as accountable care organizations, health homes, and patient-centered medical homes; and how CHWs can be financed.

The Initiative will announce plans for the next phase of their work, including the launch of a statewide community health worker association in partnership with the Community Health Worker Association of Rochester and the Community Health Worker Network of NYC, which will unite the state’s regional CHW networks. The statewide association will play a leading role in providing expertise, guidance and support to efforts aimed at strengthening and improving the CHW workforce of New York.
